	Permanent Residency Applicant Biographical Intake Form (Completed by the Scholar Employee)
	Request to pay outside counsel - Sample Letter
	Summary of Recruitment Sources (must provide hard copies of recruitment documents as well as proof of publication duration)

After You Submit to OIA

Once OIA receives approval from the Department of Labor, we will contact the department and the applicant about the next steps of filing Form I-140 and I-485 with the United States Citizenship and Immigration Services.
